SUPPORTING DOCUMENTATION


MEDICAL FORMS

INCLUSION SUBSIDY SUPPORT FORMS

ISS forms must be completed for any child with a diagnosis of having an additional need (eg. ADHD, Asperger’s, Autism, etc.). The Health Care information form must be completed by a medical professional (GP, Psychologist, Pediatrician, etc.)
The remaining two forms are to be completed by the family.
Children with additional needs will not have a confirmed booking until all three forms have been submitted (minimum 3 weeks prior to program)

ISS forms must be completed for ANY child with a diagnosis of having an additional need (eg. ADHD, Asperger’s, Autism, etc.).
The two forms here must be completed and submitted with all registration/booking documentation and are to be completed by the parent/guardian of the child/ren.
ALSO REQUIRED
*A supporting letter from an Allied Health Professional, stating that the child is either undergoing an assessment, or has a diagnosis

BEHAVIOUR MANAGEMENT PLAN

To assist YMCA School Holiday Program staff to better care for your child we require as much information as possible.
If your child has an existing Behaviour Management Plan in place at school please ensure you provide a copy of this when submitting your registration & booking forms.
If your child has a history of behavioural concerns but doesn’t have a plan in place please ensure you consider completing the Behaviour Management Template as per the link below.
This does not impede your child’s chances of participation, it betters our ability to care for your child.

FEES & PAYMENTS

Fees: 

  • $69.20 per child daily fee – centre-based day
  • $97.40 per child daily fee – excursion days
  • NDIS - Educator Support: Child support ration
    1:1 $48.14 per hour +daily fee, 1:2 $26.96per hour +daily fee, 1:3 $19.19 per hour+ daily fee

The Geelong & District YMCA Vacation Care service is a Commonwealth approved childcare provider.
The program is subsidized through the Childcare Benefit Scheme (CCB). Daily fees can be reduced by applying Child Care Benefits, please see the ready reckoner link below to assist you to estimate your fees.

Child Care Subsidy (CCS)
Child Care Subsidy (CCS) will be paid directly to the approved child care provider/s (YMCA GEELONG) to reduce the fees you pay once FAMILIES REGISTER (with Centrelink via MY GOV accounts). In order to have benefits applied this important step needs to be complete before booking your child into our program. Full fees will apply if you have not authorised Geelong and District YMCA Vacation Care through the MYGOV portal.

The Governments Child Care Subsidy system unfortunately deactivates families with approved child care provider/s after 8 weeks. As due to school holidays falling out of the 8 week period for CCS, families will need to visit their Centrelink online account through myGov, or the Express Plus Centrelink app to AUTHORISE Geelong and District YMCA Vacation.

This issue has been raised with the Government authorities as it is an issue across all service providers.

HOW TO BOOK INTO OUR PROGRAM

  1. Submit your School Holiday Program booking request form to the YMCA Geelong(via email This email address is being protected from spambots. You need JavaScript enabled to view it. or drop off at YMCA Newtown Stadium Reception 25 Riversdale Road, Newtown).
  2. Allow up to one week from the date submitted for our School Holiday Program Co-ordinator to process your request.
  3. Log into your Mygov account and select Geelong and District YMCA Vacation Care as your authorised Provider.
    If Geelong and District YMCA Vacation Care does not come up as a provider on your screen, there are a few reasons this will occur;
    a. We may not have processed your booking request form yet
    b. We need to un-cease your account and re- submit your family details as a customer of our service
    c. You have not provided the Geelong and District YMCA Vacation Care with the correct CRN and date of birth for Guardian/Parent and Child/ Children. Please make sure that when you submit a booking request that you provide the CRN of the guardian/parent that is linked to the child/rens CRN.
  4. If you cannot select Geelong and District YMCA Vacation Care on your Mygov account please contact This email address is being protected from spambots. You need JavaScript enabled to view it. immediately.
  5. Please re-check your Mygov account at a later date.
  6. A confirmation of your booking request occurs when we issue the Guardian/Parent with a Statement. PLEASE Note: CCS may not have been applied at this time.
    Please check that the dates are correct.
  7. A re-issued Statement ( with CCS for eligible families) will be sent to Guardian/Parent two (2) business days prior to scheduled Direct Debit.
  8. PLEASE NOTE: IF YOU DO NOT APPROVE Geelong and District YMCA Vacation Care as your provider prior to every school holiday booking you will be direct debited the full fees. Parents will then be required to organise the CCS directly with CentreLink. We do not have authority to organise the CCS on your behalf.

Grandparent Child Care Benefit

You will already have received a letter from Centrelink outlining the new evidentiary requirements for the Additional Child Care Subsidy (grandparent). You will need to go online to provide your Child Care Subsidy information and apply for Additional Child Care Subsidy (grandparent) – this is when the documentary evidence to confirm your relationship to your grandchild and that you have 65% or more care of your grandchild will be required.
Jobs, Education and Training Child Care Fee Assistance (JETCCFA)
You will need to complete an online assessment for Child Care Subsidy. You will then receive a second letter from Centrelink in June 2018 setting out how to apply for Additional Child Care Subsidy (transition to work).
Special Child Care Benefit (financial hardship)
You will need to complete an online Child Care Subsidy assessment. If you are assessed as eligible for the Child Care Subsidy, you will be able to apply for Additional Child Care Subsidy (temporary financial hardship) online from 2 July 2018.

It is highly important that families provide the relevant information to YMCA School Holiday Program in order for fees to be properly calculated. We require all children BIRTH DATES and CUSTOMER REFERENCE NUMBERS (CRN's). We also require the BIRTH DATE AND CRN of the parent who is linked to the child/ren.

PAYMENT OF FEES

All families will be required to submit a Direct Debit Form (see further down).
Families will receive an invoice/statement as confirmation of tentative booking prior to each program. This will detail the service fees as well as list an estimate for any CCS
Please note that invoices/statements generated prior to each program are estimates based on your eligibility for Child Care Benefits & Child Care Rebate. An invoice/statement with calculated fees can only be produced after attendances have been submitted after the program has concluded.

Families may notice that there are slight differences between the estimated fees and the calculated fees. This is not in the control of YMCA Geelong and information is calculated based on each individual family’s CCS. The Child Care Management System cannot always produce an accurate figure until after the program and once the attendances have been logged. Any outstanding fees should be paid promptly.
If a credit remains families can choose to leave the credit until next using the program, transfer the credit to other YMCA program fees, or request a refund by submitting in writing individual bank information.

DIRECT DEBIT

All families are required to submit a Direct Debit form as per the links below for access to YMCA Geelong’s School Holiday Program.
